On today’s show we will be speaking with Amy Devine, the Community Liaison for Hospice of Havasu. We will be speaking with her about the new Transitions program they are offering. Transitions services are administered by social workers and volunteers. They provide:
- Assessment of client needs and referrals to appropriate community services and agencies
- Help in understanding and completing advance directives
- Examination of long term care and care-giving options
- Guidance in discussing health concerns and feeling related to life-limited illness
Transitions is a non-medical service offered at no charge or obligation.
Hospice of Havasu is a not-for-profit, patient-centered team of professionals and volunteers, who serve the physical, emotional, and spiritual needs of the incurably ill, their families and friends in our communities.
